Material Failures? We see often in product failures that the original specifications for the materials were correct, but that the life and use of the product exceeded the material's usage specifications. Substantive Failures? A substantive failure would be a case where the original design is not adequate–either in the strength and amount of the materials, or simply the choice of material. A product design review can establish post failure design fault.
